The Community Health Needs Assessment is a primary tool used by Northside Hospital to determine its community benefit plan, which outlines how it will give back to the community in the form of health care and other community services to address unmet community health needs.
The assessment is comprised of primary data collection and secondary data analysis that focus on the health and social needs of the service area.
The Patient Protection and Affordable Care Act and IRS section 501(r)(3) require that tax exempt hospitals conduct a Community Health Needs Assessment and develop an Implementation Strategy every three years.
