Sponsorship Application Requirements

  • Requests must come from a certified 501(c)3 corporation or other charitable organization under the U.S. Internal Revenue Code
  • Requests must include a completed Charitable Giving Application along with supporting documents.
  • Event requests must include details regarding when and where the event will take place as well as details regarding sponsorship benefits. 
  • Requests must be submitted at least two months in advance of the event or activity.

Sponsorship benefits should include, but are not limited to the following: Northside Hospital employee engagement, volunteer opportunities, incorporating a Northside Hospital rep as part of programming and social media recognition.

Sponsorship request for individuals, scholarships, religious or political purposes, and organizations other than those granted 501(c)(3) status by the IRS will not be considered.

How to Apply

1. Download & Complete Your Application

Download the Charitable Giving Application. Note several supporting documents are required to complete the application process. Please include all documents when submitting your application. Applications must be signed and dated in the current year in order to be accepted.

2. Submit your completed application.

Please email your completed charitable giving application to sponsorships@northside.com. Be sure to include supporting documents such as sponsorship benefits and details regarding the date, time, and location of your event.
